Complications we would run into if we use perturbation theory without having made a multipole expansion first would be finding the first order perturbation hamiltonian H_1. We use the multipole expansion to seperate the interaction hamiltonian into a sum of different orders where we use the first order correction of the interaction hamiltonian as the first order perturbation of the system.
