When you put an observer next to the slits to see which slit is taken the wavefunction of the quantum system will collapse to one of its eigenstates. It will not be in a superpostion anymore. We will see two lines with the same itensity and no interference pattern. The amplitudes of the two possible paths is equal. therefore the intensity of both lines will be the same. There will be no fasefactor.
